IMPORTANT HEALTH COVERAGE TAX DOCUMENTS: FORM 1095-C

Rutherford County 2025 1095-Cs are now available. If you opted into electronic delivery through MyBenefitsChannel, you can view, download, save, and print your form by logging in and, in the Communications Center, selecting View 1095-C Forms.

Your electronic consent carries forward from year to year. If you opted in to receive your 1095-C electronically last year, that consent rolls over to this year.

You can follow this link at any time to view your electronic consent status and opt in or out of future electronic 1095-C delivery. Please be aware that opting into or out of electronic delivery now will not impact the delivery of your 2025 1095-C, but will be held on record for the delivery of your 2026 1095-C in 2027.

If you did not opt in to receive your 1095-C electronically, your 1095-C will be available upon request. This is a change from previous years, in which all 1095-Cs that were not electronically delivered were mailed to employees, and is allowed by updated federal regulations.

HOW TO REQUEST YOUR 1095-C

Full-time and ACA-eligible part-time employees may request a copy of their Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, by contacting the Insurance & Risk Management department at:

We will furnish the requested form within 30 days from our receipt of your request. If you request a copy of your form and have opted-in to electronic delivery, we will deliver it via the Secure Inbox of MyBenefitsChannel.

If you request a copy and have not opted in to electronic delivery, a paper copy will be mailed to your address on file or be available to pick up in Insurance & Risk Management office at the address above, Monday through Friday, 8:00am to 4:30pm (excluding government holidays). If you are picking up in person, we advise you to call ahead so we can prepare your form and have it waiting for you when you arrive.
